Hannah’s House is a registered NDIS provider and is required to participate in regular audits as part of our ongoing commitment to high-quality, safe, and person-centred care.
As part of this process, external auditors may need to review participant files and speak with clients or their representatives to ensure our services meet the NDIS Practice Standards. Participation in this process is completely voluntary, and your decision will not affect your supports or services in any way.
Please read the following information carefully before signing your consent.
If you agree, you are consenting to any or all of the following:
- Auditors reviewing your client file (e.g. service agreements, progress notes, assessments, reports).
- Auditors contacting you or your nominated representative via phone call for an interview on June 10th or 11th 2025.
- The information you provide being used by the auditors to assess the quality and safety of our services.
All information shared with auditors is handled confidentially and in line with the NDIS (Quality Indicators) Guidelines 2018 and relevant privacy legislation.
