We encourage families, staff, and community members to share their feedback or report concerns confidentially.
How to report an incident
As an NDIS provider, we have a responsibility to effectively manage and respond to incidents and complaints. If you witness or become aware of an incident that affects a child’s safety or wellbeing, it is crucial to report it immediately.
A reportable incident is any act or event that has happened—or is alleged to have happened—in connection with the delivery of NDIS supports or services that:
- Results in the death or serious injury of a child.
- Involves abuse, neglect, or sexual misconduct.
- Includes unauthorised use of restrictive practices.
Registered NDIS providers must notify the NDIS Commission of all reportable incidents, including those recorded and addressed through internal incident management systems.

Internal Incident Reporting
- These reports help us identify risks, ensure accountability, and take preventive measures.
- An incident is an unplanned event directly involving an organisation which results in or had the potential to result in: injury or harm to one or more people, business interruption, or equipment, property, environmental damage.
- Incident reports should be submitted promptly through our internal reporting system.
External reporting
Certain incidents must be reported to external authorities, including the police, child protection services, and regulatory bodies.

Mandatory reports can be made orally or in writing, but an oral report must be followed up in writing as soon as possible (ideally within 24 hours).

If you are dissatisfied with a health, mental health, or disability service (excluding NDIS services), you can lodge a complaint with HADSCO (Health and Disability Services Complaints Office).
Certain serious incidents must be reported to the NDIS Commission within 24 hours. These include:
- Death or serious injury of a child.
- Abuse or neglect.
- Unlawful sexual or physical contact.
- Sexual misconduct, including grooming.
- Unauthorised use of restrictive practices.
