Individualised Care

We conduct individual health assessments in collaboration with the school and your family. Based on these assessments, we develop tailored care plans. Our Hannah’s House staff then undergo rigorous training and competency assessments to ensure they meet the child’s clinical and individual support goals safely while at school.

We'd love to work with your school!

Our team is comprised of dedicated Registered Nurses and Disability Support Workers. They work under the guidance of our Complex Care Coordinator (Schools) to provide expert care for children with complex health needs while they’re at school.

We currently support many children with complex health needs attending Education Support Schools and have existing agreements with:

  • Burbridge School
  • Durham Road School
  • Castlereagh School
  • Carson Street School
  • Kalamunda Primary Education Support Centre

We are here to help your child and family
