Support for Self-Managed National Disability Insurance Scheme (NDIS) and WA NDIS

Hannah’s House is working toward our formal registration as an NDIS service provider, but we are already providing and can offer supports to families with self-managed NDIS funding across metro Perth.

Hannah’s House specialises in the provision of in-home supports to children from birth to 18 years, with an emphasis on younger children. Depending on the agreed requirements of your Plan, Hannah’s House can offer:

  • Support with daily life tasks to offer families and carers respite, training and education and assist with your child’s development goals and life skills [Cluster 2 supports – NDIS WA]
  • In-home respite and assistance to allow parents and carers to have a break or undertake typical family activities [Cluster 5 supports – WA NDIS]

Hannah’s House typically works with families who have children living at home with life-limiting conditions and complex needs. Please review our information for Families and Referring Agencies for a more detailed overview of the types of children we may be able to support.

As a small provider, you will find engaging Hannah’s House offers unique advantages. You will always have a single point of contact directly with our Clinical Services Manager. We understand how frustrating the process can be and we aim to keep things simple with hassle-free support.

We also understand that your funding may not give you as much care for your child as you had hoped. Our goal is to work within your Support Plan and maximise the benefit of your funding through highly competitive rates for Registered Nurses or experienced Disability Support Workers.

Hannah’s House is a recognised service provider to Princess Margaret Hospital. We take referrals and have provided both in-home respite and in-home palliative care to children and families through PMH. You can feel secure in the knowledge that we will always provide you with professional, caring and family-centred support.
