Make a Referral

Referrals are made to Hannah’s House by Perth Children’s Hospital, family GPs, medical specialists, community nurses and social workers, disability specialists, support coordinators and Plan Managers Families, you can also make a direct referral for programs.

If you are an agency, the family must have given you their consent to a referral being made.

Hannah’s House provides care to children and young people aged between the ages of 0-18 years.

Our criteria for providing services either fee-free or through the NDIS, is that the child has a diagnosis of a progressive, life-limiting or other condition that results in high intensity or complex care needs.
