By uploading images, I consent to, on behalf of my child /or myself to Hannah’s House:
Using, re-producing, communicating, printing, exhibiting or distributing the Materials (full or in part) in all formats and all media known currently or that may be devised in future.
Use of Materials I agree that:
The Materials may be edited at Hannah’s House sole discretion;
I or my child is not entitled to any remuneration for the use of the materials;
I or my child will not seek copyright ownership of the materials;
Hannah’s House may use my child’s name, likeness, and the Materials to promote itself.
Acknowledgement of Use I acknowledge that this agreement permits Hannah’s House to use the Materials in all forms of media including, without limitation:
In Hannah’s House printed materials such as its annual reports, brochures, newsletters, and flyers;
On Hannah’s House website, Facebook, YouTube, Instagram, or other social networking sites; and
In videos, television, and other forms of electronic material.
Revocation of Consent
Consent to Hannah’s House use of the Materials may be revoked upon my providing 1 month’s written notice to Hannah’s House.
If my consent is withdrawn, I acknowledge that Hannah’s House is under no obligation to destroy, delete, withdraw, or recall the Materials in existence at the date my consent is withdrawn.
Hannah’s House agrees that it will not be entitled to use, reproduce, communicate, exhibit or distribute the Materials (in full or in part) in any format or media created by Hannah’s House subsequent to the revocation of consent after the 1 month notice period.
Release: On behalf of my child, I agree to release Hannah’s House from any claim by me or my child arising out of the Materials and/or my child’s appearance in promotional campaigns in which the Materials are used. Collection of Personal Data: I authorise Hannah’s House on my behalf and on my child’s behalf to collect and process our personal information held in connection with our dealings with Hannah’s House and in accordance with Hannah’s House privacy statement which can be found at www.hannahshouse.org.au Miscellaneous: I confirm that I have full authority to enter into this agreement and I understand the nature and subject matter of this consent and the manner in which the Materials are likely to be used.
